How is PCOS Diagnosed?
To be diagnosed with PCOS, you need at least 2 out of 3 of the following:
Hyperandrogenism
High testosterone levels or visible signs like:
Hirsutism (excess hair growth on face, chest, or back)
Moderate to severe acne
Irregular Menstrual Cycles
Infrequent, irregular, or absent ovulation, leading to cycles longer than 35 days to no periods at all.
Polycystic Ovarian Morphology (PCOM)
Seen on ultrasound (≥20 follicles per ovary or ovarian volume ≥10 cm³) or high AMH levels (Anti-Müllerian Hormone, is a hormone produced by small follicles in the ovaries)
Note: For adults, if irregular cycles and hyperandrogenism are present, ultrasound may not be needed.
For teens, diagnosis is based on irregular cycles and signs of hyperandrogenism only - as it's normal for adolescent ovaries to have a higher number of follicles, which can appear 'polycystic' on ultrasound.
Did You Know There Are Different PCOS Types?
Not all PCOS is the same! Here are the subtypes:
Insulin-Resistant PCOS – The most common (up to 70%). High insulin drives up androgens → irregular cycles, acne, hair growth & more.
Post-Pill PCOS – Temporary symptoms after stopping hormonal birth control due to hormonal rebound.
Inflammatory PCOS – Chronic inflammation contributes to hormonal and metabolic issues.
Adrenal PCOS – Elevated adrenal androgens (DHEA-S), leading to acne or hirsutism—often without cysts.
